Return to School waiver

We are looking forward to welcoming those students who chose to return back to our campuses starting on October 22, 2020. If your children are a part of the hybrid instructional model, please sign the attached waiver form and return it prior to your child's start date.

You can return a signed copy in the secure drop box located in front of your child's school, scanned back via email to your school using one of the addresses below, or you can contact your school if you need additional assistance getting this form signed and returned prior to your child coming on campus.

It is important that we receive this signed form as we cannot allow students on campus without it.
